Clinical Observations on Paget's Disease of the Breast

Abstract
Even though Velpeau first desribed the nipple symtoms of Paget's disease and James Paget issued the first report that the nipple symtoms were related to breast carcinoma the arguments still continue about the definition and the origin of this
disease.
With the clinical reports of 15 patients who had operations in Kangnam St. Mary's Hospital from July 1981 to June 1993, the authors observed their frequency and sign, onset duration, diagnostic method, operation method, pathologic findings, and
survival. Nine out of 15 cases revealed the possibility of follow up.
@ES The results were as follows:
@EN 1) The incidence of Paget's disease was 2.8% (15/541)
2) Chief complaints include nipple erosion (5 cases), nipple ulcer (2 cases), nipple discharge (5 cases), mass (3 cases), mass with nipple symptom (9 cases). There was no preponderance in site. The lesion was confined to the nipple without
evidence of
malignancy within the breast in one patient.
3) Average illness period was 20 months ranged from 3 months to 8 years.
4) Mammogram showe the positive result in 3 out of 7 cases. One out of two cases showed the positive result on MRI.
5) Simple mastectomy (2 cases), simple mastectomy with axillary lymph node dissection (1 case), modified radical mastectomy (11 cases), radical mastectomy (1 case) were performed as operation methods.
6) We could observe 7 cases of intraductal carcinoma and 7 cases of invasive ductal carcinoma in the breast. Among the cases in which mass was felt (12 cases), 6 cases of intraductal caicinoma and 6 cases of invasive ductal carcioma were
observed.
All
3 cases without palpable breast mass is not related with axillary lymph node metastasis.
7) Three out of nine possible follow up cases survived over 5 years. Two patietns who had invasiveductal carcinoma without axillary metastasis died within two years after the operation. Four patients who have had operations since 1992 are being
observed regularly at the out patient departement without evidence of relapse.
